Case Management

Our Case Management Program helps clients with serious, persistent mental illness make informed decisions regarding opportunities and services, and coordinates all services to meet individual needs. Case Management also ensures that the individual is linked to the services detailed in the case plan, advocates for the client when appropriate, and monitors the plan of care as well as the overall status of the individual in response to their changing needs.

Case Management Program is completely voluntary.  It is a client driven program that uses resources to assist clients restore or learn new skills. Clients must meet certain criteria to be eligible for case management services.

Case Managers work Monday through Friday, 8 a.m. to 4:30 p.m.  Referrals come from psychiatrists, nurses, family members, hospital, medical doctors, and other agencies. The case manager has the primary responsibility for implementing and assuring that appropriate case plans are developed to meet the individual’s needs.
