Integrated Health Home

The Vera French Integrated Health Home Program (IHH) consists of a team of professionals working together to provide whole-person, patient centered, coordinated care for adults diagnosed with Serious Mental Illness (SMI) and children with Serious Emotional Disturbance (SED). Care coordination is provided for all facets of a person’s healthcare. This includes physical health, mental health, dental, vision and preventative care. The team will assist in getting the services and support each individual needs to lead a healthier and happier life.

Caring for the Whole Person

Vera French Community Mental Health Center now offers an exciting program that embraces all facets of your healthcare. This includes physical health, mental health, dental, vision and preventative care. We will assist in getting the services and support you need to lead a healthier and happier life.

What are the benefits?

The program helps you create a plan for medical care and wellness goals. The plan, and your team, will focus on your choices, your strengths, good health and total wellness.

The program will also include:

• Help with managing your physical and behavioral health services.
• Transportation to healthcare appointments if needed.
• Support Services provided to you and your family members.
• Access to an IHP team member 24 hours a day, 7 days per week.
• Technology to link all your services together.
• A team working to promote your recovery, resiliency and independence!

How will this affect me?

Instead of just treating a single aspect of your health such as depression or diabetes, you and your team will focus on how your mind, body, choices and behaviors influence your wellness.

How do I join?

To join the Vera French Integrated Health Home Program there are two requirements:

1. You must be eligible for Iowa Medicaid

2. Have a serious mental illness that negatively impacts your life

Do I have to join?

No, but if you choose not to participate, you could lose eligibility for some services; such as targeted case management.

You can arrange all of your health services at Vera French Community Mental Health Center if you wish or you can get your health services anywhere that accepts Medicaid.
