Habilitation Home Program

Overview

The Vera French Habilitation Home Program offers support for individuals facing mental health challenges to enhance their capacity for independent living within the community. This program is structured to address a wide range of needs by facilitating the development of essential life skills, promoting self-assurance, and assisting participants in reaching personal objectives. Services are centered on strengthening daily living, social, and coping skills, thereby enabling individuals to maintain or advance their ability to reside successfully in the community.

Services

Habilitation Home services are provided up to 24 hours per day within a group home environment. Services are individually tailored to the needs of each client, but may include:

  • Adaptive Skill Development: Building skills needed to function independently and successfully in various environments.
  • Assistance with Activities of Daily Living: Supporting the development and maintenance of essential skills for independent living, including personal care, meal preparation, housekeeping, and medication management.
  • Community Inclusion: Opportunities and assistance to build social connections, engage in recreational activities, develop personal growth, and access community resources, all while promoting independence and integration.
  • Transportation: Assisting in getting to and from appointments, activities, and community settings.
  • Adult Educational Support: Supports such as benefits education, career exploration, employment assessments, and job skills training.
  • Social and Leisure Skill Development: Opportunities to engage in group activities, hobbies, and community outings that encourage socialization and personal growth.
  • Personal Care: Assistance with daily routines such as bathing, grooming, dressing, and hygiene, ensuring dignity and comfort.
  • Protective Oversight/Supervision: Compassionate supervision to ensure safety and well-being at home and in the community.
  • Coordination of Services: Collaboration with families, medical professionals, and other service providers for holistic care and seamless transitions.

Habilitation Homes

Vera French operates nine (9) staffed Habilitation Homes in Davenport, Iowa, designed for individuals requiring additional support and supervision. Each residence accommodates 4 to 5 individuals. The group home environment facilitates the development and reinforcement of essential skills such as personal care, household management, employment acquisition and retention and adaptability to new circumstances, thereby equipping residents to successfully participate within both home and community settings.

Vera French services are delivered by highly trained, compassionate professionals. Our staff is dedicated to fostering respect, independence, and a positive environment for everyone we serve.

Program Eligibility

You may be eligible if both of the following are true:

  • you are eligible for full Medicaid, and your income is at or below 150% if the federal poverty line
  • If you are only on a Medicare Savings Program, you are not eligible for Habilitation Services
  • You need HCBS to live and work successfully in the community. Vera French will review information about your needs, your past experiences and your income to decide if you are eligible for the program.
